In order to be seen by a healthcare provider or to be treated in our facility, please call Student Health Services at 239-590-7966.
Students may schedulde certain types of appointments through their Student Health Portal.
If you need an STD/STI, women's health, birth control, pregnancy test, or concussion evaluation appointment please call.
You may also come in and schedule an appointment that meets your needs.
Student Health Services is adjacent to Parking Garage 2, behind the Arts Complex.

Reminder:
- You must bring your Eagle ID with you for all appointments.
- Missed appointments not canceled at least 2 hours ahead of the appointment time are subject to the $25 no-show fee.
- Bring a form of payment for any nominal charges you may incur at the time of your appointment. Student Health Services takes all major forms of payment including Visa, Mastercard, American Express and Discover. Your Eagle ID can also be used for payment if there are existing funds associated to the card; charges also can be posted to your Gulfline account.
Class Absence Request Form
Unforeseen events or circumstances may occur that cause a student to be absent from class. These could include illness, medical issues, bereavement, accident, hospitalization, pregnancy and/or parenting, or a catastrophic event. Please use the link below to fill out the class absence notice request form from Student Care Services. Please note that this form in not reviewed after-hours, on the weekends, or during university holidays or breaks. If you need to report an emergency, call the University Police Department at (239)590-1900, or dial 911.

FGCU does not offer a domestic insurance plan. There is an international student plan that is coordinated by the Global Engagement Office. Please click the following link for updated information regarding the FGCU insurance plan: Academic Health Plans
All visits to Student Health Services, regardless of insurance coverage, are free. Some medications, labs and supplies are provided at nominal prices. Students need to bring some form of payment to each appointment to cover the possibility of a charge.
Billing Information
Check-out Information: Not all visits incur charges; however, if your provider recommends labs or medications, there will be charges that are incurred at nominal rates. Any charge(s) incurred at Student Health Services must be paid for at the time of service in the form of credit card, debit card, Apple pay, or Eagle ID Dollars. Please bring a form of payment for your visit.
No-Show Fee: Students who incur the $25 fee can pay directly at Student Health Services. This will allow them to avoid the financial hold from Student Account Services. Students are granted 1 business week from time of charge to pay; failure to complete payment at Student Health will result in the charge being transferred to their FGCU account.

We understand that medical information about you is personal. We are committed to protecting medical information about you. Throughout this Notice, we will refer to your "protected health information" as your PHI.
We create a record of the services you receive at FGCU SHS to provide quality care and to comply with certain legal requirements. This notice applies to all of the records, electronic or paper, of your care generated by SHS practitioners. We may not disclose any more of your PHI than is necessary.
We are required by law:
- To make sure that your PHI is kept private.
- To inform you of our legal duties and your rights in regards to privacy practices with your PHI.
- To follow the terms of the notice that currently is in effect.

HIPAA
In 1996, the Health Insurance Portability and Accountability Act (HIPAA) was endorsed by the US Congress as a result of the American public's concern about how patient health information was stored, accessed, disclosed and shared. The HIPAA Privacy Rule provided the first nationally-recognized and enforced set of regulations concerning the use and disclosure of what is commonly called “protected health information.”
HIPAA requires all healthcare professionals and those with access to patient data to operate within strict standards to safeguard protected health information. The US Department of Health and Human Services enforces HIPAA law and imposes penalties on institutions that do not make good-faith efforts to ensure the privacy and security of protected health information.
Student Health Services (SHS) operates in strict adherence to HIPAA laws and regulations. HIPAA requirements are incorporated into policies and procedures department-wide, and are a vital, required component of new employee and annual staff refresher training.
Included in Notice of Privacy Practices is detailed information about how patients can:
· Obtain a copy of their health information
· Request SHS amend health information in their patient records
· Receive an accounting of certain disclosures made of their health information
· Request SHS restrict the use and disclosure of their health information
· Request information regarding how SHS may contact them about medical matters
